You can register for monthly webinars and meditation moments by visiting our Strive Library. Here, you can not only register for upcoming webinars and meditation moments, but also view previous ones, view our newsletters, and access other additional wellness resources.
Strive Wellness Portal
You can participate in wellness challenges, complete your annual health risk assessment (HRA), find educational resources, track your healthy behaviors, sync your fitness device, find the latest wellness news, and more! Access the wellness portal at www.kapnickstrive.com.
From any internet browser, go to www.kapnickstrive.com.
- Select "Sign-Up"
- Enter: Last Name, DOB, & Last 4 of SSN
- Agree to Terms & Conditions
- Enter remaining information & create your username and password
Follow the instructions here to sync your smartwatch.
- From your phone, go to your App Store and search “MyWellApp by Corehealth” to download the app.
- From an internet browser, log in to your wellness portal and hover your mouse over your profile picture to reveal the drop-down menu.
- Click "Mobile Access" and then click "New."
- Enter the code into your MyWellApp, then wait 2-5 minutes before logging in.

From the Home page of your Wellness Portal click on the circle titled “Social.” From this page, scroll down until you can view the Event Calendar. On the calendar you’ll be able to view upcoming challenges, wellness presentations, webinars, and more! Additionally, from your Health Hub you’ll be able to access a printable version of the 2024 Wellness Calendar.
You can track any wellness related activities, such as 5k’s, triathlons, marathons, volunteering, going to a financial wellness class, working with a therapist, reading health and wellness-related articles, and more.
For more details about wellbeing activities and points, visit the Home page of your Wellness Portal and scroll down to the “Points Program” widget.
From the Home page of your Wellness Portal, scroll down until you see a section titled “Click the Image to Join the Challenge!” Simply click the image and click “Join Challenge” when prompted.
When completing the forms for different exams and/or vaccinations, please ensure that they are dated separately. For example, if you completed two vaccinations on the same date, each vaccination needs to be dated differently to submit both forms; i.e. only one form can be submitted per day in each category.
You will receive points on the date the form is submitted to the Wellness Portal.
Health Screenings
Participation in health screenings is important for personal well-being and helps identify potential health risks early on. It allows for preventive measures and promotes a healthier lifestyle.

- Preventive Care: Regular screenings promote a proactive approach to healthcare by helping individuals become aware of their health status and encouraging them to adopt healthier lifestyle choices.
- Cost Savings: Detecting health problems early can lead to more affordable treatment options and may reduce healthcare costs in the long run.
- Personalized Health Planning: Screening results can provide valuable insights into individual health risks and enable personalized health planning, including tailored preventive measures and interventions.
- Health Education: Health screenings often come with educational resources and information on how to manage health risks effectively, empowering individuals to take control of their health and make informed decisions.
- Overall Health Improvement: Regular health screenings contribute to overall health improvement by promoting early detection, preventive care, and health-conscious behavior.
The baseline tests included in your health report include height, weight, hip and waist measurements, blood pressure, total cholesterol, HDL, LDL, triglycerides, glucose, Hemoglobin A1C, Nicotine, and Gamma Glutamyl Transferase (GGT).
The annual biometric health screening is offered free of charge by your employer. Depending on your screening, you may have the option to pay for additional testing. For more information, please reach out to your Wellness Coordinator or HR contact.
Seven to ten business days after completing your screening, your health report can be accessed on your wellness portal at kapnickstrive.com. From the Home page, click on the circle labeled “Assessment.” From here you’ll be able to view any current and past health reports. Your report will contain your personal health score, a number derived from only your biometric screening results on a scale from 0-100, with 100 being the optimal health score. The information obtained is strictly confidential and will not be shared with anyone.
The health report can take up to 7-10 business days to be available within your wellness portal. If it has been past the 10 business days and you still have not yet received your health report, please reach out to your wellness coordinator.
A business day, also commonly referred to as a working day, is a day of the week when businesses and financial institutions are open and operational. It is typically Monday through Friday, excluding weekends (Saturday and Sunday) and public holidays. Kapnick Strive business days are Monday through Friday, 8:30 AM – 5:00 PM EST.
Yes, all information gathered from the Health Risk Assessment, Wellbeing Activity forms, and biometric Health screening is confidential. No personal health information is shared with your employer or third parties.
If your spouse is covered on your medical plan, they are eligible to have a wellness account on kapnickstrive.com and participate in the health screening. Please contact your Wellness Coordinator to see if your spouse is eligible for a wellness account.
Each individual that is eligible to participate in the health screenings has their own wellness portal. The health report will be available 7-10 business days after your biometric screening. Each individual will need to log into their own account to view their health report. In accordance with HIPPA, because the wellness portal contains personal health information (PHI), we cannot give account information to anyone other than the individual without written consent.
Although we commend your well-being efforts and support a relationship with your healthcare provider, we are unable to accept blood work received from healthcare provider.
If your healthcare provider deems it medically inadvisable for you participate in the health screenings, please reach out to your Wellness Coordinator to learn more about our medical necessity review.
Incentive eligibility criteria may vary. Your Wellness Coordinator should provide information on the specific requirements to qualify for incentives. Typically, it may involve the following:
First-time participants are automatically eligible for the wellness incentive upon completion of their health risk assessment (HRA) AND biometric health screening.
Repeat Participants that have completed a screening in the previous year will need to achieve:
- Health Score of 70 or higher OR
- Improve their previous Health Score by 5 points
- Complete their HRA
If a repeat participant does not receive a Health Score of 70 or higher and/or does not improve your previous year score by 5 points, they are still eligible to receive the incentive by completing our Reasonable Alternative Standard (RAS) program. This is a telephonic health coaching program in which you will speak with a health coach over the phone once a week for 6 weeks to review your health results and create a plan to achieve your health and wellness goals.
If you believe there's an error in your health report, contact Kapnick Strive at Strive@kapnick.com to report the discrepancy.
RAS Health Coaching Program
A health coach is a professional who supports and guides individuals in achieving their health and wellness goals. They assist with creating personalized strategies for improving overall well-being, focusing on areas such as nutrition, exercise, stress management, and lifestyle changes. Health coaches provide motivation, accountability, and expert advice to help make sustainable changes and reach desired outcomes.

Upon immediate completion of the RAS program, the wellness coordinator will reach out to your employer and notify them you have completed the RAS program. You then will become incentive eligible.
In addition, you are now equipped with valuable tools and resources to seamlessly integrate wellness practices into your daily life. The newfound knowledge and skills obtained during the RAS program empower you to proactively pursue and achieve your health and wellness goals. Whether it's adopting healthier habits, managing stress, or incorporating fitness routines, you now have the means to make informed choices that contribute to your overall well-being. The post-RAS phase is not just about completion; it's about embracing an approach to health that extends beyond the program, fostering lasting positive changes for a healthier future.
Yes, if your employer provides one of the following programs, participants may enroll in one of these programs in lieu of completing the RAS program with Kapnick Strive in order to receive the wellness incentive:
- Omada
- Livongo
- Livongo Whole-Person
Participants who choose to enroll in one of these alternative programs will need to fill out the RAS Exempt form to confirm their enrollment and email it to their wellness coordinator.
