EMPLOYEE + SPOUSE COVERAGE: If you cover a spouse on the medical plan, both you and your covered spouse must participate and meet all requirements to receive the incentive. If one person does not complete the requirements, neither person will receive the incentive.
NEW HIRES OR LIFE STATUS CHANGES: You are eligible to participate if your hire date or life status change was on or before August 31, 2026, and your medical benefits started on or before September 1, 2026.
